VOLUNTEER AGREEMENT: As a volunteer, I acknowledge that I am fully aware of any and all risks posed by these volunteer activities and that I have no medical condition that prevents me from engaging in them. I am voluntarily participating in the Volunteer Activities with knowledge of the danger involved and I agree to accept all risks of participation. I release and hold harmless of DC Jazz Festival from any and all claims, costs, suits, actions, judgments or expenses upon any damage, loss or injury to me or to my belongings which may arise from this volunteer event. I also give permission to be photographed by project partners or the media for use in printed materials, through the internet or through other media outlets.

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the state in which the Volunteer Activities take place and agree that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.

 

I acknowledge that DC Jazz Festival has not arranged and do not carry any insurance of any kind for my benefit or that of Volunteer (if Volunteer is under 18). I also understand that this document is a contract which grants certain rights to and eliminates the liability of the Organization.
